Operation Accreditation Manual (PDF)

The manual includes all the information you need to know about the before, during, and after the application process and how to navigate the program properly.

Professional Inspection Form (Fillable Word Document)

A PVM Inspector, an ACCT Certified Professional Inspector, or a licensed engineer must fill out this form for your Inspection. It needs to be from within the last 364 days.

Training Affidavit (PDF)

Any training that is conducted needs to have a Qualified Course Professional fill out a corresponding Training Affidavit Form. You will need to include affidavits from the last 5 trainings.

Operation Agreement (PDF)

Agreements and Attestations between the Operation and ACCT.

Accreditation Application (Jotform)

When you have completed your Inspection, Training Affidavits, and Operation Review, you will fill out this application and submit it to the ACCT Program Manager.
